无“苏康码”应用条件的儿童、老人、海外归国人员及其他因客观原因确实无法申领“苏康码”人员，可填写《个人健康状况申报卡》，由所在单位、社区或相关管理人进行健康风险等级判定并盖章，《个人健康状况申报卡》有效期14天，和“苏康码”具有同等效力。
